Liability Waiver

This liability waiver applies to: Group Walk-and-talk therapy, Full Moon Celebrations, and other events & groups which are held in person or are held in outdoor spaces.
If you participate in any of the above groups or events, you understand and agree to the following:
That there are risks associated with any general outdoor activity, that you’re willing to assume these risks, and that I (Wellness with Jenn, LLC, Jennifer Sowers, and my associates) are not liable for such risks. Hazards may include stumbling on uneven surfaces, bee stings, sunburn, twisted ankle, etc.
That you have no known health problems or medical conditions which could in any way limit your ability to safely participate, and that you assume all health risks associated with this activity. That because groups and event sessions are outdoors, there’s some risk to confidentiality, including but not limited to the possibility of encountering a person one of us knows, some of our conversation may be overheard by someone, or that someone may recognize me as a mental health professional. You will agree to keep all members information confidential.
That I will be acting as a mental health professional under the scope of my mental health license—not as a fitness trainer or in any other capacity.
That you certify you have adequate insurance to cover any injury or damage you may experience while participating in walk-and-talk sessions, or that you agree to bear the costs of such injury or damage.
That you certify that you are aware of the health and safety guidance issued by the CDC and local authorities regarding wearing masks and maintaining a safe distance. Knowing these guidelines, you are responsible for making your own decision about whether or not to wear a mask and whether or not to participate in group, and you accept any and all risk that may come with your decision, knowing that you cannot control other people’s decisions. You waive potential liability of group leaders and participants.
That you certify you understand the risks associated with eating or drinking (before, during, or after events; at the event site) and you take full responsibility for your actions, decisions, and possible consequences. You understand that the group leaders are not nutritionists or chefs, cannot provide advice on your decisions of what to eat or drink, and are not responsible for identifying allergens or other risks on your behalf. You certify you understand potential consequences. You waive potential liability of group leaders and participants, before, during, and after the event/group/etc.
That you certify that you understand that these events/groups/etc are not designed for people with substance abuse issues, eating disorder issues, or currently in an abusive/domestic violence relationship.
That you will keep all group members information shared during the groups and event session confidential
That payments will be made upon registration, in the format you choose.
You understand and voluntarily agree to release, waive, and agree not to sue Jennifer Sowers, Wellness with Jenn, LLC, any co-facilitators, or any staff member under their charge, for any and all claims, damages, costs, attorney's fees, or causes of action which you have or may in the future, as a result of damages or injuries relating to the participation or travel to and from outdoor therapy/groups/events, arising out of or incident to any negligent act or omission by Jennifer Sowers, Wellness with Jenn, LLC. You knowingly give up valuable legal rights, including the right to sue.
By signing up for the event or group, you are verifying that you have read and agree to the terms above.
